Pre-Visit Forms
All patients are required to complete pre-visit forms, and there are three options for doing so:
- Online (recommended): Click the box below to complete the forms online. Only do so once you’ve scheduled your scan. Please arrive 15 minutes before your appointment.
- Download and print: Use the links on this page to download and print the forms. Complete them and bring them with you when you arrive. Please arrive 15 minutes prior to your scan.
- In-person: Complete the forms at our center. Existing patients should arrive 15 minutes before their scan, while new patients should arrive 30 minutes early.

Scan Preparation Information:
- MRI and MRA: Inform us if you have metallic implants. The procedure takes 20-45 minutes. You’ll hear knocking sounds, and headphones with music are available. Contact us if you require sedation.
- PET/CT: Do not eat or drink (except water) for 6 hours before your appointment. Stay hydrated and follow dietary instructions if diabetic. The scan takes about 20-30 minutes, but expect to stay for 2-3 hours total.
- CT & CTA: If contrast is required, do not eat or drink 4 hours prior. The exam takes 10-30 minutes.
- Ultrasound: Specific preparations depend on the type of ultrasound. Abdominal ultrasound requires fasting for 6-8 hours, while pelvic or obstetrical ultrasounds require a full bladder.
- X-ray: No preparation is needed. Remove jewelry or metal objects before the exam, which typically takes less than 20 minutes.
